This tool and our inbox are not monitored for emergencies. In the UK, call 999 if life is at risk or you cannot keep someone safe. Outside the UK, use your local emergency number. See urgent and non-urgent support routes ↗

Pause. Notice. Choose one next step.

These five original prompts are optional. They do not measure your mental health, assess risk or decide what support you need. Your answers stay in this page and disappear when you leave or reload. Do not include private details about other people.

NHS Every Mind Matters

Its free Mind Plan asks five questions and offers practical suggestions. Intended for adults; it does not replace professional advice. Answers are handled by the external service.

Recognising concerns in sport

The IOC Sport Mental Health Recognition Tool 2 is for athletes and the people around them. It helps recognise concerns and encourage support, not diagnose them. Developed in the elite-sport context; the separate clinical assessment tool is for qualified health professionals.
